The Basics

What is a mental health superbill?

A superbill is an itemized document a therapist can give to a client after providing services. It usually includes information about the provider, the client, the services provided, the diagnosis, and what was charged or paid.

Clients may use that document when asking their insurance plan about out-of-network reimbursement. The exact process varies by plan.

For the client’s side of the process, read our guide to superbills and therapy reimbursement.

A superbill is not quite the same thing as an invoice or receipt. An invoice mainly shows what a client owes, and a receipt shows what they paid. A superbill includes additional provider and service information that may be useful when a client submits documentation to an insurer.

Superbill Studio creates the document for you. It does not submit claims or superbills to insurance companies.

Example

What does a therapy superbill look like?

There is no single visual format every therapist has to use. A good superbill simply organizes the important information clearly so the client and insurance plan can understand what services were provided.

Anatomy of a Superbill

What does a therapy superbill usually include?

Insurance plans can ask for different information, but most mental health superbills follow the same general structure.

Provider information

Provider name and credentials
Your name as clients and insurance plans would recognize it.
Practice name and address
The practice the services were provided under.
National Provider Identifier (NPI)
Commonly included, and often the first thing a plan looks for.
Federal Tax ID (EIN)
Used to identify the practice for billing purposes.
Provider signature and date
Shows who prepared the document and when.

Client information

Client name
The person who received the services.
Date of birth
Often requested so a plan can match the document to the right member.

Session information

Date or dates of service
One appointment, or several on the same document.
CPT or HCPCS code
The code for the service provided.
Modifier, when applicable
Added when something about the service needs to be noted, like telehealth.
ICD-10 diagnosis code
The diagnosis the services were provided for.
Place of Service
Where the session happened, such as an office or telehealth.
Charge
What was billed for the session.
Amount Paid
What the client paid.

Totals

Total charges
The sum of the sessions listed.
Total amount paid
What the client paid across those sessions.

Single or Multiple Sessions

One superbill can cover more than one appointment.

Some clients need a superbill after every appointment. Others ask for one every few weeks or months. In that case, several dates of service can be listed on the same superbill instead of creating a separate document for each session.

Superbill Studio supports both Single Session and Multiple Sessions. If appointments happen weekly, recurring dates can be added quickly instead of entering every date by hand.

Templates Work. There's Just an Easier Way.

You can build a superbill manually. Or you can just make the superbill.

There's nothing wrong with making a superbill in Word or Google Docs. Plenty of therapists do. The downside is having to update the same details, check the math, fix the formatting, and export a new PDF each time.

Word / Google Docs

  1. 1Create or find a template
  2. 2Enter provider information
  3. 3Add client and session details
  4. 4Format the service rows
  5. 5Calculate the totals
  6. 6Check everything over
  7. 7Export the document to PDF
  8. 8Repeat the process next time

Superbill Studio

  1. 1Save your provider information once
  2. 2Start with your usual session details already filled in
  3. 3Add one session or several
  4. 4Let the totals calculate automatically
  5. 5Add your practice logo and accent color
  6. 6Download the finished PDF

Superbill Studio handles the repetitive part, so creating the document feels more like a quick admin task than a formatting project.
